2005 Annual Meeting Registration Form
 
Please register for the annual meeting of The John Whitmer Historical Association beginning Thursday, September 29th through Sunday, October 2nd, at the Renaissance Springfield Hotel (701 East Adams Street, Springfield, IL 62701).
 
Conferees should make their own hotel reservations by calling 888-236-2427 or 217-544-8800. Be sure to ask for the John Whitmer Historical Association rate.
 
Give the name of each person registering in the space below. Each person must have his / her own name tag as identification for admittance into the sessions. Name tags and program materials will be in the registration packets provided upon your arrival.
 
We cannot process credit cards on jwha.info, so this form generates a printable Annual Meeting Registration form which you can mail with your check to JWHA.

PLEASE NOTE: Reservations deadline is September 7, 2005. It will be necessary to have this information by that date for reservation confirmations for the caterers. After 9/7/2005 registration will cost $60 and the Presidential Banquet will cost $40.
